Job summary

Hartford HealthCare at Home is seeking a Quality Assurance & Performance Improvement (QAPI) Specialist to strengthen clinical documentation and outcomes through a data-driven approach. You will compile trends, report quality data across patient care, safety, risk, infection control, outcomes and customer satisfaction, and lead performance improvement plans.

The role requires 3 years of certified home health experience, knowledge of PDGM and value-based purchasing, and the ability to travel to

Qualifications

• 3 years of certified home health experience required.
• Experience with data analysis and statistical processes in quality reporting.
• Familiarity with Value Based Purchasing, PDGM, CAHPS preferred.

Responsibilities

• Develop and implement performance improvement plans.
• Conduct clinical record audits and provide educational feedback.
• Review data and ensure regulatory compliance across settings.
• Coordinate with comprehensive quality improvement and reporting teams.
• Facilitate PI teams using quality improvement methodologies.
• Travel to branches (regions) as needed; travel within the United States may be necessary.
