Job summary

Hartford HealthCare at Home is seeking a QAPI Specialist to advance clinical documentation and quality outcomes. You will compile trends, report data across patient care, safety, risk, infection control, and satisfaction, and develop performance improvement plans.

Qualifications

• Experience in providing data analysis and statistical processes in quality reporting.
• Ability to design and develop performance measures.
• Experience in providing education.
• Experience in facilitation of PI teams using quality improvement methodology.
• Ability to effectively communicate both verbally and in writing.

Responsibilities

• Assist with process toward improving clinical documentation and outcomes using a data-driven approach.
• Compile trends and report quality data in patient care, safety, risk management, infection control, outcomes and customer satisfaction.
• Lead development of Performance Improvement Plans based on audit results.
• Develop and evaluate clinical quality improvement action plans and interventions.
• Review OASIS assessments for appropriateness, completeness, and regulatory compliance.
• Ensure ICD-10 coding accuracy related to patient conditions and co-morbidities.
• Coordinate with quality improvement teams and educate staff on best practices.
• Travel to branches as needed to support regional quality initiatives.
