Qualifications

• 3 years of certified home health experience required.
• Value Based Purchasing, PDGM, CAHPS-preferred.
• Licensure, Certification, Registration
• Knowledge of federal and state regulatory requirements.

Responsibilities

• Assist with process improvement plans and performance improvement projects.
• Audit clinical records, track data, and report quality indicators.
• Develop and implement improvement interventions to boost quality scores.
• Coordinate with staff to ensure regulatory compliance and timely data submissions to payors.
