Job summary

Hartford HealthCare at Home seeks a QAPI Specialist to lead data-driven quality improvement across clinical documentation, patient safety and outcomes. You will compile trends, audit results, and develop performance improvement plans while ensuring regulatory compliance and education for staff.

The role requires several years in home health quality, knowledge of OASIS, ICD-10 coding, and strong communication. Travel to branches nationwide may be necessary.

Qualifications

• Experience in data analysis and quality reporting.
• Ability to design and develop performance measures.
• Experience in education and staff training.
• Facilitation of PI teams using quality improvement methods.
• Strong verbal and written communication skills.
• Willingness to travel to branches as needed.

Responsibilities

• Assist with process improvement and data-driven quality initiatives.
• Compile trends and report quality data on patient care, safety and satisfaction.
• Lead development of Performance Improvement Plans based on audits.
• Develop and evaluate Clinical Quality improvement action plans.
• Participate in QAPI activities including incidents and infection tracking.
• Ensure compliance with federal/state regs and coding accuracy (ICD-10).
• Review OASIS assessments for completeness and compliance.
• Promote evidence-based guidelines to reduce practice variation.
• Coordinate data submissions to payors and regulators.
