Los Alamos Wellness & Rehabilitation seeks an experienced MDS Coordinator to manage timely completion of the RAI and care management processes from admission to discharge, following company policy and federal/state guidelines.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, you may coordinate information systems operations and staff education within the clinical department.

Qualifikationen

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Aufgaben

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure Decisive Interdisciplinary team decisions on MDS and related assessments based on clinical criteria.
• Coordinate daily stand-up meetings, review resident care, and set assessment reference dates.
• Comply with federal and state regulations for MDS processes.
• Monitor MDS and care plan documentation in medical records.
• Maintain current MDS status per state and federal guidelines.
• Enter resident information into MDS programs accurately.
• Code MDS data from medical records and staff/family interviews.
• Attend interdisciplinary meetings to communicate changes and update records.
• Assist with identification of significant changes and ensure MDS reflects those changes.
• Prepare scheduling and notices for care planning conferences; communicate outcomes to staff and residents.
• Keep knowledge base up-to-date on data entry and computer technology.
• Submit required documentation to the state database and other entities per policy.
• Assign and instruct staff in RAI Process, PPS Medicare, Medicaid, and related clinical systems.
