Los Alamos Wellness & Rehabilitation is seeking an MDS Coordinator to oversee timely MDS and care management from admission to discharge in line with policy and Federal/State guidelines. The role coordinates education for the clinical department under the Director of Nursing and VP of Clinical Reimbursement.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process, OBRA regulations and MDS.
• Knowledge of the care planning process and MDS tools.
• Experience with MDS 3.0 preferred.

Responsabilités

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on completing additional MDS or assessments based on clinical criteria per RAI Manual.
• Coordinate daily stand up meetings to review resident care and assessment reference dates.
• Ensure compliance with federal and state regulations regarding MDS processes.
• Monitor MDS and care plan documentation in the medical record to support coding.
• Maintain current MDS status and data entry in appropriate systems.
