Job summary

SNF Staffing Solutions is seeking an MDS Coordinator to oversee timely completion of the RAI and care management processes from admission to discharge, aligned with company policy and Federal, State, and Certification guidelines.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, the role may coordinate information systems operations and education for the clinical department. Performs data entry and ensures accurate MDS coding.

Qualifications

• RN licensed and ready to practice.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, PPS Medicare, and Medicaid reimbursement.
• Understanding OBRA regulations and the MDS process.
• Knowledge of care planning and MDS 3.0 preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on completing or not completing additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings and set assessment reference dates.
• Ensure compliance with federal and state regulations for the RAI process.
• Monitor MDS and care plan documentation in the medical record to support coding.
