Job summary

Socorro Wellness & Rehabilitation seeks an experienced MDS Coordinator to ensure timely, accurate completion of the RAI process and care management from admission to discharge in compliance with federal and state guidelines. This role collaborates with the Director of Nursing and VP of Clinical Reimbursement to optimize care documentation and outcomes.

Responsibilities include coordinating the interdisciplinary team, monitoring MDS status, and ensuring proper data entry into MDS systems while

Qualifications

• RN or LVN graduation and license in the state of practice.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement, plus OBRA regulations.

Responsibilities

• Coordinate MDS/RAI process from admission to discharge in line with policies and guidelines.
• Monitor MDS coding accuracy and ensure documentation supports coding.
• Manage scheduling for care planning conferences and communication of outcomes to staff and families.
• Attend interdisciplinary team and QA meetings to update records and communicate changes.
• Lead staff instruction on RAI processes and clinical computer systems.
