Job summary

Forest Park Nursing & Rehabilitation is seeking an experienced MDS Coordinator to ensure timely completion of the RAI process and care management from admission to discharge in line with policy and regulatory guidelines.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, you will coordinate MDS coding, assist with education for the clinical team, and collaborate with the Interdisciplinary Team to review assessments and care plans.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Knowledge of MDS 3.0 and care planning processes.
• Understanding of OBRA regulations and Medicare/Medicaid requirements.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments.
• Ensure regulatory completion of the RAI process and care plans.
• Coordinate daily stand-up meetings and review assessment references.
• Maintain MDS status and accurate data entry in computerized systems.
• Code MDS with data from records, observations, and interviews.
