Job summary

Socorro Wellness & Rehabilitation is seeking a qualified MDS Coordinator to oversee timely MDS activities, care management from admission to discharge, and compliance with Federal and State guidelines. The role reports to the Director of Nursing and supports the VP of Clinical Reimbursement in education and systems operations.

Responsibilities include coordinating interdisciplinary MDS assessments, maintaining data accuracy in MDS programs, and ensuring care plans reflect changes in resident

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process and OBRA regulations.
• Knowledge of the care planning process and MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Assist in deciding when to complete additional MDS assessments based on clinical criteria.
• Coordinate daily stand up meetings to review resident care and set assessment reference dates.
• Ensure compliance with federal and state regulations for MDS processes.
• Monitor MDS and care plan documentation in the medical record to support coding.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data into MDS programs accurately and timely.
• Complete accurate coding of MDS from chart review, observations, and interviews.
• Attend interdisciplinary meetings, QA meetings, and communicate changes.
• Assist DON with significant changes, physician orders, and verbal reports to reflect MDS/care plan updates.
• Prepare scheduling and notices for care planning conferences and communicate outcomes to staff/residents.
• Attend weekly Medicare/Medicaid/Managed Care meetings as required.
• Correct and submit final MDS data to State and Federal agencies.
• Instruct staff on the RAI Process and related Medicare/Medicaid processes.
• Maintain confidentiality of necessary information.
