Job summary

OPCO Skilled Management is seeking a Nurse MDS Coordinator to ensure timely and accurate completion of the RAI and care management processes from admission to discharge, in line with company policy and federal/state guidelines.

The role collaborates with the Director of Nursing and VP of Clinical Reimbursement to coordinate information systems and education for the clinical department, ensuring high-quality documentation and compliance.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process. Knowledge of OBRA regulations and MDS.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Responsibilities

• Coordinate and complete the RAI process and care management from admission to discharge per policy and guidelines.
• Schedule and conduct MDS assessments and care plan reviews with the interdisciplinary team.
• Ensure documentation supports MDS coding in the medical record.
• Maintain current MDS status and data entry in computerized systems.
• Prepare scheduling, notices, and communicate outcomes to staff and residents/families.
• Maintain confidentiality of information and comply with federal/state regulations.
• Assist with identifying significant changes and physician orders impacting MDS and plans.
• Attend interdisciplinary meetings to gather information and update records.
