Job summary

SNF Staffing Solutions is seeking an MDS Coordinator to ensure timely, accurate completion of the RAI and care management process from admission to discharge in accordance with policy and federal/state guidelines.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, you may coordinate information systems operations and education for the clinical department. RN licensure required, SNF experience preferred.

Qualifications

• RN license required.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions on completing MDS/assessments follow clinical criteria per RAI User’s Manual.
• Coordinate daily stand up meetings and set assessment reference dates.
