Job summary

Outfield Healthcare Partners is seeking a dedicated MDS Coordinator to ensure timely, accurate completion of the RAI process and care management from admission to discharge in line with federal, state, and certification guidelines. The role works under the Director of Nursing and VP of Clinical Reimbursement to coordinate information systems and education for the clinical department.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions on completing additional MDS assessments based on clinical criteria per MDS User's Manual.
• Coordinate daily stand-up meetings to review resident care and schedule assessment reference dates.
• Maintain MDS status and ensure documentation supports MDS coding in the medical record.
• Maintain confidentiality of resident information and comply with federal/state regulations.
