Job summary

Outfield Healthcare Partners seeks an experienced MDS Coordinator to oversee timely and accurate completion of the RAI and care management process from admission to discharge in line with policy and regulatory guidelines.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, you will coordinate interdisciplinary assessments, ensure data integrity in MDS systems, and support care planning across the facility to optimize reimbursement and resident outcomes.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Ensure that the Interdisciplinary team makes decisions for MDS completion based on clinical criteria.
• Assist with coordination and management of daily stand-up meetings and assessment reference dates.
• Comply with federal and state regulations regarding MDS processes.
• Monitor MDS and care plan documentation and ensure accuracy in medical records.
• Maintain current MDS status according to state and federal guidelines.
• Enter resident data into MDS software accurately.
• Complete coding of the MDS from medical records and interviews.
• Attend interdisciplinary meetings to share information and update records.
• Support DON with changes reflected in MDS and care plans.
• Update knowledge base related to data entry and computer technology.
• Submit required documentation to state databases per policy.
• Ensure final MDS completion and submission to appropriate agencies.
• Train staff on the RAI Process and related clinical systems.
• Maintain confidentiality of resident information.
