UnitedHealth Group in Massachusetts seeks an RN Field Clinical Care Coordinator to support an Integrated Care Model. You will relay member needs, advocate for optimal care, and ensure access to necessary services.

The role is field-based with at least 50% travel to members' homes, plus telecommuting options. A MA RN license and 2+ years of clinical experience are required, with a strong emphasis on care coordination and working with diverse communities.

Qualifications

• Current and unrestricted independent licensure as a Registered Nurse in MA.
• 2+ years of clinical experience.
• Intermediate MS Office proficiency (Word, Excel, Outlook).
• Access to a quiet home workspace with ability to protect PHI.
• Ability to travel within designated regions to visit members in homes or other settings.

Responsibilities

• Engage members in face-to-face or telephonic assessments of medical, behavioral, functional, cultural, and socioeconomic needs.
• Plan and implement individualized care strategies at the least restrictive level of care.
• Identify and initiate referrals for social services, including financial and psychosocial supports.
• Manage the care plan as a single point of contact across the care continuum.
• Collaborate with internal care teams, providers, and community resources to implement care plans.
• Communicate required health information to stakeholders to ensure timely, coordinated care.
