UnitedHealth Group in Bridgewater, MA is seeking an RN Clinical Care Coordinator to be the primary care manager for a panel of members with complex medical and behavioral needs, focusing on care coordination to promote appropriate utilization of services and improved quality of care.

This full-time, day-shift role requires residency in Plymouth County and surrounding areas, up to 75% local travel, and involves field-based work with a home-based office; compensation ranges between $29 and $52 per

Formación

• High School Diploma/GED required.
• 2+ years of clinical experience required.
• Current and unrestricted RN licensure in MA.

Responsabilidades

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans addressing chronic health conditions and medication management.
• Partner and collaborate with internal care teams, providers, and community resources to implement care plans.
• Provide education and coaching to support member self-management of care needs.
• Support proactive discharge planning and manage care transitions following ER visits or admissions.
• Advocate for members and families to ensure their needs are represented by the healthcare team.
