UnitedHealthcare is seeking an RN Field Clinical Care Coordinator to manage a panel of members with complex medical and behavioral needs in Dedham, Needham, West Roxbury and surrounding areas. This field-based role offers a home-based office and up to 75% travel with mileage reimbursed.

Responsibilities include needs assessments, care planning, education, and care transitions. Requires MA RN license, 2+ years clinical experience, and travel readiness.

Qualifications

• High School Diploma or GED required; MA RN license.
• 2+ years of clinical experience; MS Office proficiency.
• Access to a quiet workspace at home and reliable internet.

Responsibilities

• Engage members in needs assessments covering medical, behavioral, functional and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions and social determinants.
• Collaborate with care teams, providers and community resources to execute care plans.
• Educate and coach members for self-management and healthy lifestyle changes.
• Support discharge planning and Care Transition after ER or SNF events.
• Advocate for members to ensure needs and choices are respected by the care team.
