Job summary

UnitedHealth Group is seeking an RN Clinical Care Coordinator to manage a panel of members with complex medical and behavioral needs. This field-based role supports medical, behavioral, and social needs and requires travel within Hampden/Hampshire County, MA with remote work flexibility.

Primary responsibilities include comprehensive needs assessments, care planning, collaboration with care teams, and coaching for self-management. Local travel up to 75% is required with mileage reimbursement.

Qualifications

• High School Diploma/GED.
• Current and unrestricted RN license for MA.
• 2+ years of clinical experience.
• Proficient with MS Office (Word, Excel, Outlook).
• Access to a quiet home workspace and ability to protect PHI.
• Access to reliable high-speed internet and transportation to travel within territory.
• Valid US driver's license.

Responsibilities

• Engage members face-to-face and/or by phone to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans addressing medical, behavioral, functional, cultural and socioeconomic needs.
• Collaborate with internal care team, providers and community resources to implement care plans.
• Provide education and coaching to support member self-management.
• Support discharge planning and care transition after ER, inpatient, or SNF admission.
• Advocate for members and families to ensure needs are represented by the care team.
