You will travel to members’ homes, collaborate with care teams, and identify social service referrals while maintaining PHI security and delivering purpose-driven care across diverse communities.

Qualifications

• Current and unrestricted MA RN licensure.
• 2+ years of clinical experience.
• Proficient with MS Office (Word, Excel, Outlook).
• Home workspace with PHI security capability.
• Reliable internet connection at home.
• Willingness to travel within MA for member visits.

Responsibilities

• Conduct comprehensive needs assessments (medical, behavioral, functional, socioeconomic).
• Plan and implement individualized care strategies.
• Identify referrals to social services and community resources.
• Maintain care plans as primary contact across the continuum of care.
• Collaborate with care teams, providers, and partners to execute plans.
• Communicate health information to stakeholders for coordinated care.
