UnitedHealth Group is seeking a RN Field Clinical Care Coordinator in Medford, MA to advocate for members and coordinate services within an integrated care model. You will conduct comprehensive needs assessments, plan and implement individualized care strategies, and collaborate with internal teams and community resources to ensure timely referrals and continuity of care.

This full-time day shift role requires MA RN licensure and at least two years of clinical experience; the position offers

Qualifications

• Current and unrestricted RN licensure in MA.
• Minimum 2 years of clinical experience.
• Active MA RN license.

Responsibilities

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Assess, plan, and implement individualized care strategies.
• Identify and initiate referrals for social service programs.
• Manage the care plan throughout the continuum of care.
• Partner and collaborate with internal care teams and community resources.
• Communicate required health-related information to stakeholders.
