Job summary

UnitedHealth Group is seeking a RN Field Clinical Care Coordinator in the Boston area. The role involves in-home visits and telecommuting, with at least 50% time in the field coordinating care for members within an Integrated Care Model.

Responsibilities include conducting needs assessments, planning individualized care, arranging social services, and coordinating with care teams to ensure timely, quality services. Travel and a valid US driver's license are required.

Qualifications

• Current and unrestricted RN licensure in MA.
• 2+ years of clinical experience.
• MS Office proficiency (Word, Excel, Outlook).
• Home workspace with PHI security and private space.
• Willingness to travel within assigned regions to visit members' homes.

Responsibilities

• Engage members to complete comprehensive needs assessments across medical, behavioral, functional, cultural, and socioeconomic needs.
• Plan and implement individualized care strategies supporting least restrictive level of care.
• Identify and refer to social service programs and community resources.
• Act as single point of contact to manage the care plan across the continuum.
• Collaborate with internal care teams, providers, and community partners to execute the plan.
