Job summary

UnitedHealth Group is seeking an RN Clinical Care Coordinator to manage a panel of members with complex medical and behavioral needs. The role focuses on coordinating medical, behavioral, and socio-economic needs to improve care quality and utilization.

This field-based position involves in-home work and up to 75% local travel. Responsibilities include needs assessments, care plan development, team collaboration, and patient education to promote self-management and prevent unnecessary

Qualifications

• Must hold a current and unrestricted RN license for MA.
• Minimum 2+ years of clinical experience.
• Proficient with MS Office Suite (Word, Excel, Outlook).
• Access to a dedicated home workspace with PHI security.
• Valid US driver’s license and reliable transportation for territory travel.

Responsibilities

• Engage members to complete comprehensive needs assessments across medical, behavioral, functional, cultural, and socioeconomic areas.
• Develop and implement person-centered care plans to address chronic conditions, wellness, social determinants, and safety.
• Collaborate with internal teams, providers, and community resources to implement care plans.
• Provide education and coaching to support self-management and lifestyle changes.
• Coordinate care transitions after ER visits or SNF admissions and manage discharges.
• Advocate for patient needs and preferences within the care team.
