Job summary

UnitedHealthcare is hiring an RN Field Clinical Care Coordinator for Hampden/Hampshire County, MA. The role involves field-based care management with a home office, frequent member visits, and substantial travel within the territory to coordinate medical, behavioral, and social needs.

Telework is available when not in the field. Requirements include MA RN licensure, 2+ years clinical experience, and the ability to work with a diverse care team to implement person-centered care plans.

Qualifications

• High School Diploma or GED is required.
• Current and unrestricted MA RN license.
• At least 2 years of clinical experience.
• Must have access to a dedicated home workspace with PHI security.
• Must have reliable transportation and ability to travel within territory.

Responsibilities

• Engage members face-to-face and/or telephonically to assess medical, behavioral, functional and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, and social determinants.
• Collaborate with internal care teams, providers and community resources to execute care plans.
• Provide education and coaching to support member self-management and lifestyle changes.
• Support proactive discharge planning and coordinate care transitions after ER/inpatient/SNF events.
• Advocate for members and families to ensure needs and choices are represented.
