Job summary

Enlyte is seeking a Field Case Manager to work in a full-time, work-from-home role with regular travel to in-person patient appointments in Riverside, CA. You will coordinate care across physicians, employers, and patients, developing and implementing treatment plans to restore work capacity and cost-effective healthcare services.

The position requires an active RN license in good standing, 2+ years of clinical practice, and URAC-recognized CCM/CDMS/CRC/CRRN or COHN certifications.

Qualifications

• Active RN license in good standing.
• URAC-recognized certification in case management (CCM, CDMS, CRC, CRRN or COHN, COHN‑S, RN‑BC, ACM, CMAC, CMC).

Responsibilities

• Provide field case management for injured workers with treatment plans.
• Coordinate with treating physicians/providers, employers, customers and legal representatives.
• Assess recovery and determine occupational vs non-occupational factors.
• Collaborate with physicians/providers to tailor treatment plans.
• Modify job duties with employers to facilitate early return to work.
• Evaluate and adjust case goals based on patient progress.
• Independently manage workload and prioritize cases.
• Attend injured worker appointments when appropriate and prepare status updates.
