Job summary

St. Luke's University Health Network in New York is seeking a Clinical Triage Specialist (RN) to deliver compassionate patient care through the Access Center, triaging calls and guiding patients to appropriate services.

The CTS-RN uses nursing judgment per protocols, documents assessments in the EHR, and collaborates with POD and practice teams. A PA/NJ RN license is required; remote work options may be available with the Network Work from Home Policy.

Qualifications

• Graduate of an accredited nursing program.
• Active RN licensure in PA and NJ or other necessary states by policy.

Responsibilities

• Answers triage calls, prioritizes clinical triage per protocols.
• Verifies patient demographics and updates EHR.
• Escalates clinical issues to POD team and provides guidance per protocols.
• Handles escalated electronic messages when not on calls with clinical judgement.
• Delivers a positive patient experience and resolves issues at the point of contact.
• Meets productivity, schedule adherence and quality standards.
• Documents symptoms, nursing assessment, advice and patient response.
• Collaborates with Access Center teams and practice colleagues to address concerns.
• Other duties as assigned.
