St. Luke’s University Health Network is seeking a Clinical Triage Specialist (RN) for the Access Center to deliver compassionate patient care and provide clinical support to team members.

The RN will triage calls using established protocols, document findings, and ensure appropriate care referrals while maintaining a focus on patient experience and safety. Requires active RN licensure in PA/NJ or a compact state and a minimum of two years in clinical settings such as physician offices, home

Qualifications

• Active RN licensure in PA or NJ or compact state.
• Minimum 2 years recent clinical experience in a physician office, home health, critical care or emergency room.
• Strong communication and customer service skills.
• Focus on compliance and continuous growth.
• Ability to work from home per Network policy.

Responsibilities

• Answer telephones, triage calls, follow protocols, and coordinate services.
• Verify demographics and accurately enter/update information into the EHR.
• Escalate clinical issues and provide guidance per protocols.
• Manage escalated electronic messages when not on calls; prioritize and refer as needed.
• Deliver a positive patient experience and resolve issues at the time of the call.
• Meet productivity, schedule adherence, and quality standards as set by the Access Center.
• Use guidelines to assess, prioritize, advise, schedule, or refer calls to appropriate personnel.
• Document symptoms, nursing assessment, advice provided and patient/caller response.
• Collaborate with Access Center teams and practice clinicians on behalf of the patient.
• Other duties as assigned.
