Option Care Health, Inc. is seeking a Clinical Transition Educator to provide clinical education and training for smooth patient transitions to our care model.

You will partner with the sales team to increase patient transitions to our home infusion services, delivering education to patients, caregivers, and facility staff. The role requires RN licensure, healthcare experience, and strong presentation skills, with 100% local travel to care facilities and hospitals to support timelines and

Qualifications

• Active and unrestricted RN license required.
• Minimum 2 years healthcare industry experience.
• Willingness to obtain nursing licensure in additional states if needed.

Responsibilities

• Evaluate, educate, and train patients, caregivers, and facility staff on OCH services for in-home or alternative-site care.
• Conduct patient assessments to determine eligibility for the OCH care delivery model.
• Partner with the sales team to identify educational needs and improve patient transition volume.
• Maintain confidentiality of patient information and follow legal guidelines.
• Document education activities using OCH applications and technology.
• Coordinate transitions from hospital to home with facilities, providers, and internal teams.
• Provide in-person education and collect pre-admission and insurance information.
