Job summary

Option Care Health is hiring a Clinical Transition Educator to provide clinical education and training for patients transitioning to our home care model. You will partner with the sales team to grow patient transitions and ensure seamless, safe care at home.

The role requires an active RN license and at least 2 years in healthcare, with strong presentation and documentation skills. Travel is local, with some in-person work and facility visits for education and transition support.

Qualifications

• Active and unrestricted RN license.
• Minimum of 2 years in healthcare.
• Ability to build relationships at all levels of an organization.

Responsibilities

• Educate patients, caregivers, and facility staff about OCH services and products for successful transition.
• Assess patient viability for the OCH care model.
• Coordinate with sales to identify educational needs and improve transition volume.
• Maintain confidentiality of patient information and company data.
• Document education using OCH applications and technology.
• Coordinate transitions from hospital to home or alternate care settings.
• Provide in-person education and gather pre-admission and insurance information.
