Job summary

Option Care Health, Inc. is seeking a Clinical Transition Educator who will educate patients, caregivers, and facility staff on home and alternate-site infusion services, ensuring smooth transitions to our care model.

You will partner with the sales team to identify educational needs, coordinate with hospitals and care facilities, and document activities using OCH applications while maintaining patient confidentiality.

Qualifications

Active and unrestricted RN license required.
Minimum 2 years healthcare industry experience.
Ability to build relationships at all levels of the organization.
Experience applying standard practices (ACHC, URAC).
Strong customer service experience with quality standards.
Proficient PC skills: internet, email, Word, Excel, PowerPoint.
Must have reliable transportation for local travel.
Willingness to obtain nursing licensure in additional states as needed.
Able to plan, organize, and give presentations.

Responsibilities

Educate patients, caregivers, and facility staff about OCH services for home or virtual care.
Conduct patient assessments to determine eligibility for the care model.
Collaborate with sales team to align educational needs and grow transition volume.
Maintain confidentiality and adhere to HIPAA; protect proprietary information.
Document education using OCH applications and technology.
Coordinate transitions from hospital to home with facilities and care teams.
Provide in-person education and gather pre-admission and insurance information.
