Job summary

Option Care Health Inc. is seeking a Clinical Transition Educator to educate patients, caregivers, and facility staff about transitioning to our home infusion model.

You will work with the sales team to grow patient transitions and ensure smooth discharge planning and therapy education. The role requires an active RN license, strong relationship-building skills, and the ability to travel to care facilities and hospitals.

Qualifications

• Active and unrestricted RN license required.
• Experience establishing relationships at all levels of an organization and with vendors.
• Experience applying standards (ACHC, URAC).
• Experience providing customer service to internal and external customers.

Responsibilities

• Educate, train, and assess patients, caregivers, and facility staff for transition to OCH delivery model.
• Coordinate with OCH sales to identify educational needs and improve patient transition volume.
• Document education using OCH applications and technology with confidentiality.
• Coordinate transition from hospital to home with facility staff and providers for timely discharge and continuity of care.
• Provide in-person education and gather pre-admission and insurance information for initiating services.
