Job summary

Option Care Health is seeking a Clinical Transition Educator to deliver clinical education and training for patient transitions to our home infusion model. You will partner with the sales team to increase patient transition volumes while maintaining high standards of care.

The RN must educate patients, caregivers, and facility staff, conduct assessments, and coordinate with hospital teams to ensure timely discharge and safe home-based care. 100% local travel is required.

Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years of experience in healthcare.
• Ability to educate patients and caregivers on home infusion services.

Responsibilities

• Educate patients, caregivers, and facility staff about home infusion services to ensure successful transitions.
• Conduct patient assessments to determine eligibility for the OCH care model.
• Coordinate with sales to develop education for increasing patient transition volume.
• Maintain confidentiality and comply with legal guidelines.
• Document education using OCH applications and technology.
• Coordinate patient transitions from hospital to home with providers and internal teams.
• Provide in-person education and gather insurance information for service initiation.
