Option Care Health is seeking a Clinical Transition Educator in Houston to lead clinical education and patient transition initiatives. You will partner with the sales team to expand patient transitions to our care delivery model and deliver in-person education to patients, caregivers, and facility staff.

The role requires active RN licensure and at least 2 years in healthcare, with a focus on education, collaboration, and process improvement.

Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years of experience in the healthcare industry.

Responsibilities

• Educate patients, caregivers, and facility staff about OCH services and products for home or alternate-site care.
• Conduct patient assessments to determine suitability for the OCH care delivery model.
• Coordinate with the sales team to identify educational needs and improve patient transition volume.
• Maintain confidentiality of patient information and comply with legal guidelines.
• Document education activities using OCH applications and technology.
• Coordinate transitions from hospital to home or other care settings to ensure timely discharge and continuity of care.
