Job summary

Option Care Health LLC is seeking a Clinical Transition Educator to deliver clinical education and training for patient transitions to our home-based delivery model. You will partner with the sales team to drive more patients into OCH care, educate patients, caregivers, and facility staff, and ensure a smooth transition process.

• Active and unrestricted RN license.
• Minimum of 2 years of experience in the healthcare industry.
• Strong customer service and relationship-building abilities.

Responsibilities

• Evaluate, educate, and train patients, caregivers, and facility staff about OCH services and products for in-home or facility care.
• Conduct patient assessments to determine viability for joining the OCH care model.
• Coordinate with the sales team to identify educational needs and drive higher patient transition volume.
• Maintain confidentiality of patient information and comply with legal/privacy guidelines.
• Document education using OCH applications and technology accurately.
• Coordinate transitions from hospital to home or other care settings with facilities and internal teams.
• Provide in-person education, gather pre-admission and insurance information, and facilitate service initiation.
