Option Care Health, Inc. is seeking a Clinical Transition Educator to educate patients, caregivers, and facility staff on the home and alternate-site infusion care model. The role partners with the sales team to expand patient transitions to OCH care delivery.

The position requires an active RN license, 2+ years in healthcare, and strong relationship-building and presentation skills. Travel is 100% to facilities and hospitals to support education and transition initiatives.

Qualifications

• Active and unrestricted Registered Nurse (RN) license.
• Minimum of 2 years of experience in the healthcare industry.
• Experience establishing and maintaining relationships at all levels of the organization.
• Experience with standard practices for services offered (ACHC, URAC).
• Proficient PC skills: MS Word, Excel, PowerPoint.

Responsibilities

• Educate and train patients, caregivers, and facility staff on OCH services and home/in-home delivery model.
• Assess patient viability for transition to OCH care delivery.
• Collaborate with sales to identify educational needs and improve transition volume.
• Maintain confidentiality and comply with legal guidelines for patient data.
• Document education using OCH applications and technology.
