Job summary

Fallon Health's Summit ElderCare program in Webster, MA seeks a Clinic Supervisor to oversee daily clinical operations in the PACE center. You will supervise Medical Assistants and Health Aides, coordinate patient care, and collaborate with physicians and care teams to meet participant needs.

The ideal candidate is an RN with Massachusetts license, has supervisory experience with frail elders, and can manage scheduling and supply procurement within a multidisciplinary team.

Qualifications

• Must be licensed as an RN in Massachusetts.
• Graduate of an accredited nursing program.
• CPR and Alzheimer/Dementia certification preferred.
• Access to reliable transportation to perform work throughout the PACE center's service area (30-mile radius).

Responsibilities

• Recruit and train staff within the discipline including Medical Assistants and Health Aides.
• Oversight of related contracted vendors.
• Represent the program at assigned community and external events.
• Evaluate staff on achievement of goals and competencies; manage performance issues with Clinical Manager/HR.
• Provide ongoing feedback and guidance to staff.
• Conduct new staff orientation and performance evaluations.
• Coordinate provision of patient care and scheduling within the medical clinic for PACE providers and ancillary services.
• Collaborate with participants, caregivers, physicians, and other staff to meet participant needs.
• Oversee Health Aide scheduling for functional maintenance programs.
• Oversee scheduling of INR tests and community blood draws for Medical Assistants.
• Participate in Interdisciplinary Team meetings and quality meetings as required.
• Coordinate purchases of medical/surgical/pharmaceutical/food service supplies.
• Ensure clinical equipment is in good working condition with Quality/Risk.
• Lead inservice education for staff.
• Interview candidates and advise Clinical Manager on hiring; address diverse needs of participants.
• Monitor and evaluate therapeutic interventions; identify emergencies and initiate nursing orders/interventions.
• Participate in nursing on-call rotation as assigned.
• Support RN colleagues and adjust assignments as directed by Clinical Manager.
• Conduct regular staff meetings with direct reports.
• Assist with routine nursing duties as required.
