Job summary

Fallon Health’s Summit ElderCare is seeking a Clinical Supervisor to oversee daily medical operations and the ADHC program. You will lead a nursing team, coordinate care among PACE providers, and ensure adherence to policies and quality goals in a fast-paced community-based program.

The role requires RN licensure in Massachusetts, two years of supervisory experience with frail elders, and collaboration with physicians, caregivers, and staff. IV skills are a plus; CPR and dementia cert preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in Massachusetts.
• CPR and Alzheimer’s/Dementia certification preferred or willingness to be certified.
• At least two years of supervisory experience with frail elders in a health care setting.
• Ambulatory clinic experience is recommended but not essential.

Responsibilities

• Recruit and train staff within the discipline including Medical Assistants and Health Aides.
• Oversee related contracted vendors and external events.
• Represent the program and discipline at community events as needed.
• Evaluate staff performance and address issues with Clinical Manager and HR as appropriate.
• Provide ongoing feedback and guidance to staff and conduct regular orientation/assessments.
• Coordinate patient care and scheduling within the medical clinic with PACE and ancillary providers.
• Collaborate with participants, caregivers, physicians, nurse practitioners and staff to meet needs.
• Oversee Health Aide scheduling for maintenance programs and INR/community blood draws.
• Participate in Interdisciplinary Team meetings and quality reviews.
• Coordinate purchasing of supplies and ensure equipment is well maintained.
• Lead in-service education for medical assistants and health aides.
• Interview candidates and support hiring decisions.
