Fallon Health is seeking aRegistered Nurse to provide direct nursing care to Summit ElderCare (PACE) participants across settings, following nursing process and standards. You will advocate for patients, coordinate care with the interdisciplinary team, and document all changes in the medical record.

Requires MA RN license, CPR/Alzheimer’s cert, and EMR proficiency. Two years’ direct care experience, reliable transportation, and a willingness to cover varied shifts within the PACE network are

Qualifikationen

• Graduate of an accredited school of nursing.
• Current license as an RN in the state of Massachusetts.
• CPR and Alzheimer’s certification, or willingness to be certified within 60 days of hire.
• Access to reliable transportation to perform work throughout the PACE center’s service area (30-mile radius).
• At least two years of recent experience in direct care of adults or chronically disabled persons, including elderly population.
• Ability to use an electronic medical record.

Aufgaben

• Deliver direct nursing care to PACE participants in any setting.
• Advocate for the best care for each participant.
• Develop individualized plans of care and coordinate the care team.
• Communicate effectively with the healthcare team and document all health status changes.
• Participate in IDT processes and site committees.
• Perform medication administration and nursing procedures as ordered.
