Job summary

Fallon Health is hiring an RN to provide direct nursing care to Summit ElderCare PACE participants in various settings. The role emphasizes using nursing processes, individualized care plans, and collaboration with an interdisciplinary team to ensure high-quality, culturally competent care for adults aged 55 and older in Massachusetts.

The incumbent will assess, plan, implement, and document care while meeting policy guidelines and participating in training and on-call rotations.

Qualifications

• Graduate of an accredited school of nursing.
• At least two years of direct care experience for adults or chronically disabled persons; elderly population preferred.
• Competencies include customer focus, teamwork, quality improvement and action.

Responsibilities

• Reviews and implements Provider’s orders.
• Conducts nursing assessments per policy guidelines (physical, psychosocial, behavioral, MDS-HC).
• Delivers care in various settings per individualized care plans.
• Participates in the interdisciplinary team (IDT) process and care planning.
• Documents all aspects of participant care and health status changes.
