Job summary

Fallon Health's Summit ElderCare seeks an experienced Registered Nurse to deliver direct nursing care to PACE participants across settings, using nursing process and established standards. Qualifications

• Graduate of an accredited school of nursing.
• Current license as an RN in Massachusetts.
• CPR and Alzheimer’s certification, or willingness to be certified within 60 days of hire.
• Access to reliable transportation to cover 30-mile radius and possible coverage at other SE PACE centers.
• At least two years of direct care experience with adults or chronically disabled persons; elderly population preferred.
• Ambulatory care or home care experience helpful.

Responsibilities

• Reviews and implements Provider’s orders.
• Conducts nursing assessments according to policy guidelines including physical, psychosocial, behavioral, and MDS-HC as indicated.
• Delivers care to participants in any setting, based on individualized needs and nursing standards.
• Participates in the development and ongoing review of each participant’s care plan.
• Participates in the nursing “on-call” rotation as assigned.
• Documents all aspects of participant care and health status changes in the medical record.
• Assists in training and orientation of new nursing staff.
• Performs all duties in accordance with Fallon Health and Summit ElderCare policies.
• Participates in Weekend RN On Call rotation.
