Fallon Health's Summit ElderCare® is seeking an experienced Registered Nurse to provide direct nursing care to PACE participants across home and community settings in Springfield, MA. You will apply nursing process, collaborate with an interdisciplinary team, and ensure timely, culturally competent care aligned with individualized care plans.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in Massachusetts.
• CPR and Alzheimer’s certification, or willingness to be certified within 60 days of hire.
• At least two years of direct care experience with adults or chronically disabled persons.
• Ambulatory care or home care experience is helpful.

Responsibilities

• Reviews and implements Provider’s orders.
• Delivers care to participants in home settings based on care plans.
• Performs nursing assessments including physical, psychosocial, behavioral and MDS-HC as indicated.
• Documents care in the medical record and participates in IDT processes.
• Medication administration and on-call rotation participation.
• Initiates and completes falls, infection, grievance and incident reports.
