Fallon Health, Summit ElderCare in Webster, MA, seeks a Registered Nurse to deliver direct nursing care to PACE participants in diverse settings. You will assess, plan, and document care, while collaborating with the IDT to meet participant needs and ensure timely, culturally appropriate care.

The role requires MA RN licensure, CPR/Alzheimer's certification (or willingness to obtain within 60 days), and at least two years of direct adult or elderly care experience.

Formación

• Graduate of an accredited school of nursing.
• Current RN license in Massachusetts.
• CPR and Alzheimer's certification, or willingness to obtain within 60 days.
• Minimum two years of direct care for adults or elderly population; ambulatory/home care helpful.

Responsabilidades

• Review and implement provider orders and nursing plans.
• Perform comprehensive nursing assessments per policy including MDS-HC.
• Engage participants and families based on needs and abilities.
• Deliver nursing care in various settings, respecting age-appropriate standards.
• Ensure culturally sensitive care and document changes in the medical record.
• Participate in care plan development with the IDT and supervise others as needed.
• Identify emergencies and initiate appropriate nursing interventions.
• Participate in weekend on-call rotations and maintain policy compliance.
