Fallon Health is seeking a Registered Nurse for PACE participants served through Summit ElderCare. You will provide direct nursing care, conduct comprehensive assessments, and collaborate with an interdisciplinary team to implement individualized care plans.

Qualified candidates hold a MA RN license, CPR certification, and have at least two years of direct adult or elderly care experience. This full-time role offers onsite responsibilities within the Fallon Health network in Massachusetts.

Qualificações

• Graduate of an accredited school of nursing.
• Current Massachusetts RN license.
• CPR certification required or willing to obtain within 60 days of hire.
• Two years of direct care experience; elderly population preferred.
• Ambulatory or home care experience helpful.

Responsabilidades

• Reviews and implements Provider orders.
• Conducts nursing assessments per policy guidelines.
• Involves participants and significant others as needed.
• Delivers care in various settings per individualized care plans.
• Monitors and evaluates therapeutic interventions and documents changes.
• Participates in development and review of each participant's care plan.
• Provides on-call coverage as assigned.
