Job summary

Chapters Health System is seeking an RN, Case Manager to assess patient/family needs, coordinate the Plan of Care with the IDT, and provide clinical and palliative care to keep patients in their home environment.

Requirements include an active RN license in Florida, at least 1 year of nursing experience (hospice or hospital preferred), and BLS certification. Proficiency with EMR/EHR and a valid driver’s license are required. Travel and strong communication skills are essential.

Qualifications

• Active RN license in the state where employed
• Minimum of one year nursing experience; hospice or hospital preferred
• Experience with EMR/EHR systems
• Mobile driver with valid license and auto insurance
• Reliable transportation to meet visit schedule
• Ability to communicate in English (verbal and written)
• Ability to visit participants in their homes for assessments
• Ability to perform essential functions including lifting/pushing as required
• Active BLS for healthcare professionals

Responsibilities

• Provides reassurance to patients and families over the phone.
• Coordinates in-person visits when needed or requested.
• Utilizes resources or personnel to resolve complex situations.
• Documents patient/family information in the EMR and communicates with the IDT.
• Completes initial and semi-annual assessments for all services.
• Explains services to patients/families and addresses questions regarding needs.
• Initiates Plan of Care with Physicians and IDT.
• Initiates skilled nursing interventions to prevent complications and maximize comfort.
• Reports changes in patient condition to IDT or health professionals.
• Develops relationships with case managers and physicians at facilities.
• Educates patients and families about disease, care, and safety practices.
• Home visits to assess safety, medication and nutrition compliance.
• Participates in IDT meetings and quality improvement activities.
