Job summary

Chapters Health System is seeking an RN, Case Manager to assess patient and family needs, coordinate a Plan of Care with the IDT, and deliver clinical, palliative and supportive care to allow patients to stay in their homes as long as possible.

The role requires Florida RN licensure, at least 1 year of nursing experience (hospice/hospital preferred), EMR/EHR experience, and a valid driver’s license. Schedule is typically Monday–Friday with day hours, and a benefit-rich environment.

Qualifications

• Current RN license in Florida.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience using an EMR/EHR system.
• Valid driver’s license and automobile insurance.
• Reliable transportation to meet visit schedule.
• Ability to lift patients and/or equipment.
• Active BLS certification.
• Ability to visit participants in their homes.

Responsibilities

• Provide direct patient and family care as part of IDT.
• Coordinate Plan of Care reflecting patient’s evolving needs.
• Educate patients and families about disease, palliation, and safety.
• Schedule visits based on risk score and needs.
• Conduct home visits to assess safety, meds, and independence.
• Report changes to IDT and adjust care plans.
• Collaborate with physicians and staff; SBAR communication.
