Job summary

Chapters Health System in Florida is seeking an RN, Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to help patients remain in their home environment as long as possible.

You will conduct home visits, document in the EMR, and collaborate with hospitals, physicians and peers to ensure timely, high-quality care. Requires Florida RN license, active BLS, and reliable transportation.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems required.
• Valid driver’s license and automobile insurance per Company policy.
• Ability to communicate effectively in English (verbal and written).
• Active BLS for healthcare professionals from AHA or Red Cross.

Responsibilities

• Assess patient/family needs and coordinate the Plan of Care with the Interdisciplinary Team.
• Provide clinical, palliative and supportive care to patients in their home.
• Document patient/family information in the EMR and communicate with the IDT.
• Coordinate in-person visits and referrals as needed.
• Participate in Quality Improvement activities and maintain compliance with policies.
