Job summary

Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to keep patients at home where possible.

The role requires an active RN license, 1 year nursing experience (hospice/hospital preferred), EMR experience, and a valid driver's license for home visits; BLS certification is required.

Qualifications

• Current RN license in the state of employment.
• Minimum of 1 year nursing experience; hospice or hospital preferred.
• Experience working with an EMR/EHR system.
• Mobile driver with valid driver's license and automobile insurance.
• Reliable transportation to meet visit schedule.
• Able to communicate effectively in English (verbal and written).
• Able to visit participants in their homes for assessments.
• Able to perform essential functions including lifting with/without accommodation.
• Active BLS for healthcare professionals from AHA or Red Cross.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT.
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient and family about disease, care, and safety practices.
