Job summary

Chapters Health System in Florida seeks an experienced RN, Case Manager to assess patient/family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to help patients remain in their home.

The role requires current RN license, at least 1 year nursing experience (hospice or hospital preferred), EMR/EHR experience, a valid driver’s license, and strong communication to work with IDT and families.

Qualifications

• Current RN license in the state where working.
• Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
• EMR/EHR experience with Electronic Medical/Health Records.
• Valid driver’s license and automobile insurance per Company policy.
• Ability to visit participants in their homes for assessments.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.

Responsibilities

• Provides and manages direct care to patients and families as part of IDT.
• Coordinates the Plan of Care reflecting patient needs.
• Educates patients and families about disease, prevention and palliative care.
• Reports changes to IDT and other health professionals.
• Presents reports to IDT; communicates with physicians and staff.
• Supervises Home Health Aides as requested.
• May be required to report during emergencies to ensure continuity of services.
