Job summary

Chapters Health System in Florida is seeking an RN, Case Manager to assess patient and family needs, coordinate Plan of Care with the IDT, and provide clinical, palliative and supportive care to help patients stay at home.

You will use nursing process, educate caregivers, communicate with physicians, and document in EMR. Must have RN license, at least 1 year hospice or hospital experience, and active BLS.

Qualifications

• Current RN license in the state
• Minimum 1 year nursing experience in hospice or hospital
• Experience with EMR/EHR systems
• Valid driver's license and auto insurance
• Reliable transportation for visit schedule
• Effective English communication (spoken and written)
• Ability to visit patients in their homes
• Ability to perform essential functions including heavy lifting
• Active BLS for healthcare professionals (AHA/Red Cross)

Responsibilities

• Provides and manages direct care as part of the IDT
• Coordinates Plan of Care for evolving patient needs
• Educates patients and families about disease, care, and safety
• Presents reports to IDT with concise oral and written input
• Communicates with physicians and staff using SBAR
• Supervises home health aides and CHWs as requested
• Responds to emergencies and supports continuity of services
