Job summary

Chapters Health System in Cape Coral, FL is seeking an RN Case Manager for the PACE program to assess patient needs, coordinate care with the IDT, and deliver clinical, psychosocial, and palliative support to keep participants home.

You will conduct home visits, develop Plans of Care, educate families, document in the EMR, and collaborate with physicians and facilities; a Florida RN license, BLS, and 1+ year hospice or hospital experience are preferred.

Qualifications

• Current RN license in the state of Florida.
• Minimum of 1 year nursing experience in hospice or hospital setting preferred.
• Certification of Alzheimer's Disease and Related Dementias Training through the Florida Dept of Elder Affairs.
• EMR/EHR system experience is preferred.
• Active BLS certification for healthcare professionals.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT.
• Coordinates the Plan of Care reflecting patient needs.
• Educates patient and family about disease, prevention and palliative care.
• Documents patient information in the EMR and communicates with IDT.
• Develops relationships with facilities and physicians and coordinates referrals.
