Job summary

Chapters Health in Florida is seeking an RN, Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and deliver clinical, palliative and supportive care to keep the participant at home as long as possible.

The role requires an active RN license, experience in hospice or hospital care, familiarity with EMR/EHR, and a valid driver's license to visit homes as part of the IDT.

Qualifications

• Current RN license in Florida.
• Minimum 1 year hospice or hospital nursing experience preferred.
• EMR/EHR experience required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess patient and family needs and coordinate the Plan of Care with the IDT.
• Provide clinical, palliative and supportive care in the patient’s home environment.
• Perform home visits and ensure safety and quality of care.
• Communicate changes to the IDT and obtain physician orders as needed.
• Maintain documentation in the EMR per policy.
